VIS: Vaccine Information Statements

Vaccine Information Statements (VIS) are information sheets produced by the ;Centers for Disease Control and Prevention (CDC) that explain both the benefits and risks of a vaccine to vaccine recipients, their parents, or their legal representatives. Federal law requires that VIS be handed out whenever certain vaccinations are given.

In Michigan, it is important that vaccine recipients, their parents, or their legal representatives be given the Michigan versions of VIS because they include information about the MCIR. By state law in Michigan, parents must be informed about MCIR. Vaccine Information Statements that are obtained from other sources (e.g., from the CDC or IAC websites) do not contain information about MCIR.

To download the Michigan VIS, visit the MDCH Immunization VIS website.

The Tdap Vaccine Information Statement (VIS) has recently been updated and is attached.  This VIS contains information about Tdap only.

For patients receiving Td, continue using the Td/Tdap VIS dated 1/24/12 until a VIS dedicated exclusively to Td (currently in development) is available. Changes to the updated VIS relate primarily to recent changes in ACIP recommendations regarding use of Tdap during pregnancy.

The Quick Look at Using Tdap Vaccine handout, developed by MDCH staff, is available online. Quick Looks provide a quick reference, summarizing relevant information from ACIP recommendations, which can be useful when administering vaccine. Quick Look Handouts for other vaccines are posted at  www.michigan.gov/immunize > Provider Information > Quick Looks.

Providers should begin using the new VIS immediately.

Additional Information:

Dear Immunization Partners,

The PCV13 Vaccine Information Statement has recently been updated and is attached. Providers should begin using the new VIS immediately.

This update incorporates information associated with new ACIP recommendations on vaccinating adults, published in October 2012. The new VIS uses a new slightly simplified format, designed to make VISs easier to read and process, and also to standardize the format. You may notice that some types of information not relevant to the patient at the time of the vaccination (for example detailed information about indications or catch-up schedules) have been eliminated.

A second CDC document, designed as a quick reference for providers, gives additional information about the vaccine, including guidance on precautions and contraindications and links to appropriate ACIP recommendations -- information that should enable providers to answer questions patients might ask. This document is coming soon. One of these notes will accompany each VIS updated from now on.
